Paramedics say all people were declared dead when they arrived on the scene.
A grey BMW split in two when it collided head-on with another vehicle on the N17 in Secunda.
Three adults and a girl child believed to be around the age of 9 were killed.
The horrific clash happened over the weekend on Sunday.
ER24 paramedics said debris was scattered all over the area of the accident when they arrived on the scene.
The grey BMW, which was found in the middle of the road, was split in two, and the other was found off the side of the road.

ER24 spokesman Russel Meiring said the body of a man was found lying off on the side of a road “a short distance away from his wrecked vehicle”.
“In the second vehicle, paramedics discovered the bodies of a man, woman and young girl lying trapped,” said Meiring.
“Unfortunately, all four patients had sustained numerous and fatal injuries. Nothing more could be done for them and they were declared dead,” he said.
Police are investigating the cause of the accident.
